This agreement, between Hope Redefined, and the client named in this form submission will include the Initial Inventory, approximately 30 minute initial session, and 60 minute sessions. The services provided to the client include a powerful, comprehensive process for personal transformation and coaching, as designed jointly with the client. Coaching may address specific personal healing, personal projects, business successes or general conditions in the client's life. I promise that all information provided will be kept strictly confidential. I will be honest and straightforward, asking questions and making requests. The purpose of our interaction is to keep you on purpose and aligned with your intentions and to support you in realizing those intentions.
To begin our working relationship, and to maximize our effectiveness, I make the following requests:
• Respect of Time: Please make all appointments/calls on time, or notify me at least 24 hours in advance to reschedule. If a 24-hour notice is not given, the client will be billed the full rate of a session.
• Honesty: My job is to support you in those actions YOU have chosen for yourself. Please use me as a safe place to look at what stops you from being in action.
• Transparency: Sometimes I may ask questions, which may seem too probing. Look right now and decide if you can give me full permission to be bold and forthright in all our interactions.
• Commitment: Look now and see if your goal is one you are absolutely certain you are ready to begin making happen NOW.
• Ownership: Your healing and growth is solely dependent on you. The more you put in the more you will reap. Owning your process will serve you best.
• Freedom: You have the freedom to gain as much as you want from our coaching relationship. You also have the right to end our coaching agreement at any time. I ask that the termination be made in writing so that we can track the conclusion of the agreement.
Identified goals will be documented in the clients intake form and initial call with the coach. As a client, I understand and agree that I am fully responsible for my well being during my coaching calls, including my choices and decisions. I recognize that coaching is not therapy and that professional referrals will be provided if needed.
Consent to Release: Due to the nature of our support and ministry there are occasions when supporting professionals within Hope Redefined need to hand off “clients” for various other groups, coaching, retreats. As a team of professionals we are committed to protecting your confidentiality and privacy. We commit to uphold our code of ethics as professional coaches, therapists and caretakers. We commit to only share pertinent information that is needed in order for another team member and/or supporting professional to support our client(s). Information shared could be (but not limited to) coaching progress, intake forms, assessments, current emotional and health needs.
The purpose of release is coordination or transfer of care, referral to other services, consultation with other supporting professionals within Hope Redefined.
